The orthopoedic medical team at Saint Joseph’s consists of a physician, nurse, physical therapist, dietician and discharge planner. The team uses electronic medical records so each patient’s records can be accessed easily and accurately by whichever specialized physician the patient is seeing at the time. Weekly discharge planning rounds include each staff member involved with the patient's care. An orthopaedic case manager works with the patients and their families to finalize discharge plans, outlining individual needs, diet recommendations, required medications, outpatient rehabilitation arrangements and any necessary home modifications.
It has been shown that combining this process of teamwork with a whole system approach to education, rehabilitation and treatment has resulted in more successful outcomes, faster healing times, and patients getting back to maximum abilities in the shortest time possible.
